This organization, established in 1997, created the "Five Wishes" document designed to help people express how they'd like to be treated if they ever fell seriously ill and were unable to speak for themselves. Provides people with practical information, advice, and legal tools needed to ensure their wishes and those of their loved ones will be respected.
Five Wishes lets your family and doctors know: Which person you want to make health care decisions for you when you can't make them; The kind of medical treatment you want or don't want; How comfortable you want to be; How you want people to treat you; and What you want your loved ones to know.
